目的探讨多排CT低剂量不同扫描方法在新生儿颅脑检查中的应用,探讨最优化扫描模式。方法选择须接受颅脑CT扫描的新生儿200例,按照数字列表法随机分为螺旋扫描组与断层扫描组各100例,两组患者再分为A、B、C、D、E亚组各20例,管电压均设置为120kV,A组采用常规剂量100mAs,B、C、D、E组分别采用90、80、70、60mAs低剂量扫描,比较各组患儿图像质量与辐射剂量。结果 (1)断层扫描组A、B、C、D、E亚组图像评分依次减低,E组评分与A、B、C、D组比较差异有统计学意义(P<0.05),提示D组为扫描参数最佳;(2)螺旋扫描组A、B、C、D、E亚组图像评分依次减低,D、E组评分与A、B、C组比较差异有统计学意义(P<0.05),提示C组为扫描参数最佳,其图像质量评分与断层扫描D组满意率比较差异无统计学意义(P>0.05);(3)断层扫描组D组CTDIvol、ED明显低于螺旋扫描组E组,比较差异有统计学意义(P<0.05)。结论断层扫描模式检查新生儿颅脑质量良好,辐射剂量较螺旋扫描更低。 |

Objective To explore the application of different scanning method of low-dose multi-detector CT in neonatal brain examination and to explore the most optimized scanning mode. Methods 200 neonates to receive cranial CT were included and divided into spiral scanning group and tomoscan group,100 cases for each,according to random number table.Each group was then further divided into A,B,C,D and E subgroup,20 cases for each. All tube voltage were set as 120 kV,routine dose of 100 m As was adopted in subgroup A and 90,80,70,60 m As were adopted in subgroup B,C,D,E,respectively.Image qualities and radiation doses in each group were compared. Results(1)In tomoscan group,image quality decreased successively along the order of subgroup A,B,C,D and E,where there were statistically significant differences between subgroup E and subgroup A,B,C,D( P <0. 05),suggesting that the scanning parameters of subgroup D were the best;(2)In spiral scanning group,image quality decreased successively along the order of subgroup A,B,C,D and E,where there were statistically significant differences between subgroup D,E and subgroup A,B,C( P< 0. 05),suggesting that the scanning parameters of subgroup C were the best,whose image quality score had no statistically significant difference with subgroup D in tomoscan( P> 0. 05);(3)CTDIvol and ED of subgroup D in tomoscan group were significantly lower than subgroup E in spiral scanning group,the difference being statistically significant( P <0. 05). Conclusion Tomoscan mode showed good quality in neonatal craniocerebral examination and applied lower dose of radiation than spiral scanning. |
