目的分析彩色多普勒检测指标在预测早孕期α地贫胎儿中的应用价值。方法选取我院2014年7月至2015年7月收治的α地中海贫血高危孕妇204例,另取同期正常孕妇50例作为对照组,检测两组孕妇的大脑动脉血流速度、胎盘厚度和心胸比,比较不同程度α地中海贫血孕妇大脑动脉血流速度、胎盘厚度和心胸比之间的差异,通过ROC曲线比较3个检测指标预测重型地中海贫血的价值。结果重型α地贫Pt-MOM显著高于中间型α地贫组、轻型α地贫组、无α地贫组和对照组,差异有统计学意义(P<0.01);重型α地贫MCA-PSV-MOM显著高于中间型α地贫组、轻型α地贫组、无α地贫组和对照组,差异有统计学意义(P<0.01);重型α地贫CTR-MOM显著高于中间型α地贫组、轻型α地贫组、无α地贫组和对照组,差异有统计学意义(P<0.01);预测重型地中海贫血的价值顺序为:CTR>MCA-PSV>PT。结论彩色多普勒可在早期对重度α地贫胎儿进行诊断,对各指标的联合应用有助于提高其诊断价值。 |

Objective To analyze the value of color Doppler index in predicting severeα-thalassemia in early pregnancy.Methods 204 cases of pregnant women who were α-thalassemia in high-risk in our hospital from July 2014 to July 2015 were selected,another 50 cases of normal pregnant women during the same period as the control group. The detection of cerebral arterial flow velocity,placental thickness and cardiothoracic ratio of pregnant women in the two groups were made,we compared the difference of cerebral arterial flow velocity,placental thickness and cardiothoracic ratiobetween the different degree of α-thalassemia,and comparedthe value of these three indexes in predicting severeα-thalassemia through the ROC curve. Results Pt-MOM in severeα-thalassemia was significantly higher than that of intermediate type α-thalassemia group,light α-thalassemiagroup,noα-thalassemiagroup and control group,the difference was statistically significant( P<0. 01); MCA-PSV-MOM in severe α-thalassemia was significantly higher than that of intermediate type α-thalassemia group,light α-thalassemiagroup,noα-thalassemiagroup and control group,the difference was statistically significant( P<0. 01); CTR-MOM in severe α-thalassemia was significantly higher than that of intermediate type α-thalassemia group,light α-thalassemiagroup,noα-thalassemiagroup and control group,the difference was statistically significant( P < 0. 01); value sequence in prediction of severe thalassemia was: CTR > MCA-PSV > PT. Conclusion Color Doppler can diagnose the severe α-thalassemia in the early stage,and the combined use of these indexes will help to improve the diagnostic value. |
