目的高频振荡通气(HFOV)治疗常频机械通气(CMV)失败早产儿呼吸窘迫综合征(RDS)的有效性及安全性评价。方法 13例早产儿RDS经CMV失败后改为HFOV,治疗过程中监测动脉血气,记录氧合指数(OI)、动脉肺泡氧分压比(a/A)、吸入氧浓度(FiO2)及临床结果。结果 13例患儿CMV后a/A、OI与CMV前相比差异有统计学意义(P<0.05);11例存活患儿HFOV 6h PaCO2明显下降,12ha/A明显上升;OI明显下降,FiO2逐步下降,差异有统计学意义(P<0.05)。HFOV期间未出现肺气压伤,颅内出血未加重。结论 CMV失败可能加重病情,HFOV用于CMV失败早产儿RDS有一定效果,及早应用可能有利于提高抢救成功率,降低并发症。 |

Objective To evaluate the Effectiveness and safety of high frequency oscillatory ventilation( HFOV) in the treatment of preterm infants with respiratory distress syndrome( RDS) after failure of conventional mechanical ventilation( CMV).Methods 13 cases of preterm infants with RDS changed to HFOV after CMV failure.During the treatment,arterial blood gas was monitored and oxygenation index( OI),alveolar arterial oxygen partial pressure ratio( a/A),inspired oxygen concentration( Fi02),clinical outcomes were recorded.Results The difference of a/A,OI in 13 cases was statistically significant between before and After CMV( P<0. 05).After 6h of HFOV,PaCO2 of 11 survival cases decreased significantly; after 12 h the a/A increased significantly,OI decreased significantly,and FiO2 gradually decreased and the difference were statistically significant( P<0. 05).The lung injury and increased intracranial hemorrhage did not appear during HFOV.Conclusion CMV failure may aggravate the condition of preterm infant with RDS.However application of HFOV for CMV failure had certain effect and early application may help improve the success rate of rescue and reduce complications. |
