目的分析无创呼吸机治疗特发性肺间质纤维化(IPF)合并感染患者的临床效果。方法选取2014年6月至2016年12月在我院接受治疗IPF合并感染患者74例,根据治疗方法不同分为对照组和研究组,每组患者各37例。对照组患者给予常规抗感染和止咳平喘治疗,研究组患者在常规抗感染和止咳平喘治疗的基础上给予无创呼吸机正压通气治疗。对两组患者心率、呼吸频率、平均动脉压、动脉血气分析、氧饱和度及肺功能指标进行分析。结果两组患者治疗后HP、RP、MBP、PaO2、SaO2水平及血pH值均较治疗前改善,差异有统计学意义(P<0.05),且研究组患者HP、RP、MBP、PaO2、SaO2水平及血PH值改善幅度优于对照组患者,差异有统计学意义(P<0.05)。两组患者治疗前后VC、TLC及DLco水平比较,差异无统计学意义(P>0.05)。研究组患者DLco水平上升幅度大于对照组,差异有统计学意义(P<0.05)。研究组患者平均住院天数为(13.94±5.03)d,明显少于对照组患者,差异有统计学意义(P<0.05);两组患者气管插管率比较,差异无统计学意义(P>0.05)。结论无创呼吸机在治疗特发性肺间质纤维化合并感染可以稳定患者生命体征,改善肺功能,降低住院时间和费用。 |

Objective To analyze the clinical effect of noninvasive ventilator therapy on patients with idiopathic pulmonary fibrosis( IPF) complicated with co-infection.Methods 74 patients with IPF infection treated in our hospital were selected,from June 2014 to December 2016,and according to different treatment methods,the control group and the study group were divided into 37 patients per group.In the control group,patients were given routine anti infection and cough asthma treatment,and the patients in the study group were treated by noninvasive ventilator positive pressure ventilation on the basis of routine anti-infective and cough asthma treatment.The heart rate,respiration frequency,mean arterial pressure,arterial blood gas analysis,oxygen saturation and pulmonary function index were analyzed in two groups. Results The levels of HP,RP,MBP,PaO2,SaO2 and blood PH in the two groups were improved,and the difference was statistically significant( P<0. 05).The level of HP,RP,MBP,PaO2,SaO2 and blood PH in the study group were better than those in the control group,and the difference was statistically significant( P<0. 05).Comparing the levels of VC,TLC and Dlco before and after treatment in two groups,the difference had no statistically significant( P>0. 05).The level of Dlco in the study group was higher than that in the control group,and the difference was statistically significant( P<0. 05). The average hospital stays in the study group were( 13. 94 ± 5. 03) d,significantly lower than that in the control group( P<0. 05). Comparing the rate of endotracheal intubation in two group,the difference had no statistically significant( P >0. 05).Conclusion Noninvasive ventilator therapy on patients with idiopathic pulmonary fibrosis with co-infection can stabilize the patient’s vital signs,improve lung function and reduce hospitalization time and expenses |
