目的通过对四川省都江堰地区妊娠早期初产及二胎的游离三碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4)和促甲状腺激素(TSH)水平的测定,分析该地区孕妇妊娠早期甲状腺激素变化趋势,建立适用于本地区的参考范围,为临床诊治甲状腺疾病提供实验依据。方法选取2015年1月至2016年12月在我院建卡并生产的单胎健康孕妇480例,健康体检非妊娠妇女69例作为对照组,采用日本东曹公司AIA2000化学发光分析仪测定甲状腺激素水平,并进行统计学分析。结果妊娠妇女各期甲状腺激素水平均呈偏态分布,以中位数(M)及双侧限值(P2.5,P97.5)表示甲状腺激素妊娠各期的参考范围。妊娠早期TSH水平与对照组差异有统计学意义(P<0.05),18~23岁年龄组与23~30岁年龄组比较FT3差异有统计学意义(P<0.05),30~44岁年龄组与23~30岁年龄组比较FT4差异有统计学意义(P<0.05),初产组与二胎组比较FT4差异有统计学意义(P<0.05)。结论都江堰地区正常妊娠妇女的促甲状腺激素(TSH)水平与非妊娠妇女相比存在差异,不同年龄妊娠妇女甲状腺激素水平存在差异。正确把握妊娠早期甲状腺激素水平变化有助于减少防范其不良妊娠结局。 |

Objective To analyze the change trend of thyroid hormone in early pregnancy of pregnant women in Dujiangyan area of Sichuan Province,and to establish a suitable reference range for patients in this area,so as to provide experimental basis for clinical diagnosis and treatment of thyroid diseases by measuring the levels of free triiodothyronine( FT3),free thyroxine( FT4) and thyroid stimulating hormone( TSH) in the first and second trimester of early pregnancy in this area.Methods From January 2015 to December 2016,480 healthy pregnant women who built cards and gave birth to a single child in Dujiangyan people’s Hospital were selected,69 non-pregnant women as control group.The thyroid hormone levels were measured by AIA2000 chemiluminescence analyzer of the East Cao company in Japan,and the statistical analysis was carried out.Results The thyroid hormone levels in each stage of the pregnant women were all distributed in a biased state. The reference range of thyroid hormones during each stage of pregnancy were indicated by median( M) and bilateral limits( P2. 5,P97. 5).The level of TSH in early pregnancy was significantly higher than that in control group( P<0. 05); The difference of FT3 between the age group of 18 ~ 23 years and the age group of 23 ~ 30 years was statistically significant( P<0. 05); There was a significant difference in FT4 between age group of 30-44 years and 23-30 years( P<0. 05); The difference of FT4 between the first delivery group and the second birth group was statistically significant( P<0. 05).Conclusion The level of TSHof normal pregnant women in Dujiangyan area was significantly different from that of non-pregnant women.There was significant difference in thyroid hormone level among pregnant women of different age groups.Correctly grasping the chan-ges of thyroid hormone level in early pregnancy is helpful to reduce and prevent its adverse pregnancy outcome. |
